How Much Can Distribution Errors Cost Pharmacies?
Distribution errors can be extremely costly for specialty pharmacies. The IQVIA Institute for Human Data Science conducted a Biopharma Cold Chain Logistics Survey in 2019, finding that total industry-wide losses from temperature-controlled logistics failures reached $35 billion annually. This figure covers lost product, clinical trial losses, and supply chain inefficiencies.
Even on a much smaller scale, the losses can be significant. On the shipment level, a single cold chain failure can destroy an entire shipment. For example, if a refrigerator unit storing temperature-controlled medications is even a few degrees off, all of the medications inside could become unusable. Direct product losses from incidents like this commonly range from tens of thousands to hundreds of thousands of dollars, and can even climb past seven figures for high-value biologics.
But the impact of these losses isn’t just financial, patients feel it too. When distribution errors occur during the transportation of specialty medications, patients can suffer greatly. Missing or delayed specialty drugs can cause treatment delays, which can trigger flare-ups or rapid disease progression. Cold-chain storage errors can render drugs impotent, making them essentially useless. All of this creates emotional distress for patients, and can even lead to billing issues or denied insurance coverage.
So how do these mistakes happen, and how can we fix them?
How Distribution Errors Occur During Specialty Medication Transportation and Delivery
Distribution errors during the delivery of specialty pharmaceuticals occur in a number of ways, from incorrect delivery locations to cold-chain failures — and they’re far more common than dispensing errors. URAC, a healthcare accreditation non-profit, found that there were 6x as many distribution errors as dispensing errors in 2025. The most common issue: the address on the medication was correct, but the carrier still delivered it to the wrong location. Final-mile delivery, in other words, is often the pain point for specialty medication distribution.
Cold-chain failures are just as common and just as costly. An audit of the Vaccines for Children Program, conducted by the HHS Office of Inspector General, revealed serious issues within cold-chain logistics. Investigators placed calibrated temperature loggers in 45 providers’ storage units and found:
- 76% of providers had vaccines exposed to out-of-range temperatures for at least 5 cumulative hours over a 2-week monitoring period.
- None of the 45 providers met all 10 required vaccine-management categories, and only 12 out of 45 temperature logs even captured an out-of-range reading that had actually occurred.
- The most common causes were equipment-level: non-calibrated thermometers, thermometers not centrally placed in the storage unit, and improper refrigerator units.
